Reinstalling Autodesk 2011

Autodesk was installed on my work computer by IT using a CD and activated.  Two weeks later, my computer crashed (unrelated) and when IT reinstalled, the activation code did not work on the new installation. Do I need to get a new code?  Where do I get the new code?

You must have had a major hardware change, like your hard drive, motherboard, or NIC.  In that case, you need to get a new activation code from Autodesk.  You can call 1-800-551-1490 for this purpose.
