Break Into objects not working

I've run into this lately in a 2018 project where duct and pipe components that typically would "break into" the pipe or duct now do not actually create a break.  The object will size and locate as if it is inserted within the pipe/duct but does not actually break the pipe as it has in the past.  These are families that we've been using for 4+ years and never had much of an issue but on this particular job they rarely work.  Not sure if there is a known issue that would be limiting the function of these but incredibly time consuming to break pipe at each valve and reconnect.

question 1: do they work when you start a completely new project?

Thanks for the replies, I've done some messing around and looks like in a new model everything works as it should so it appears that it is just within this project file.  I'm going to create another workshared project to see if I can replicate but there are several issues that I'm seeing now. 

 

At duct connectors to Air terminals when connecting flex, a small coupling is being placed at each connection even though the sizes are the same. Still have the same breaks into issues w/ just about all duct and pipe accessories or fittings placed manually.  New sections do not show on the view they are created.  When trying to create a new design option the model crashes. 

 

We've just recently installed the update 2018.2 but I can't tell if these issues coincided with that update or not.  There's also issues w/ some of the model content duct and pipe that was drawn at odd angles (eg. 63.998deg) as early on a few grids were shifted.  The building geometry is tapering with multiple different angles at play. 

 

Our file size is ~255mb. I've done an Audit and Compact save recently but hasn't seemed to help. Looks like we'd probably need to send our model for review?

This issue might be related with mapping the phases from the arch link to the MEP model.

Thanks Pawel, I'd say you're right.  Our phases have disappeared from our model...not sure how that is possible.

Found this solution from Todd Shackelford at CAD Shack.  So far it looks like it's working...not sure on what to do w/ the DO NOT USE phase after it has served its purpose but the model is doing better.
