Is this a shared file? is it on a network drive? What method are you using to save your drawing?

This is probably not an AutoCAD issue.
It's easy enough to find out who saved the file last and when to find out where the problem lies.

Are pieces moving/rotating along with others, even though you aren't selecting them? Is automatically infer constraints turned on?

OP can't seem to answer any questions with much detail so everyone is doing the nxt best thing and throwing out guesses. Each is as valid as any other guess until the OP is able to answer questions intelligibly.
That's a matter of opinion. If alterations are being done to the file, then XREF is probably not the issue.

This statement makes me think that the minor alterations are not to XREFs, even if they are present. But, I guess I might be being presumptive.
