Is there a way to flip the Dim arrows to the outside when there is not enough room inside the extension lines?

The instance properties do not and the type properties do not reveal such an option.
Looked a bit, did a bit of experimenting. Seems that if Revit can fit the arrows in the middle, it will, if they do not fit, Revit will flip them to the outside. Not really a fix, but more of a work around. Create an arrow type that is slightly larger, assuming you are using 1/8" you can create maybe a 5/32" arrow. Create a second dimension type similar to the first, using this larger arrow head. Use the new dimension style where the arrows are very close.

In the type properties, whenever you use an arrow tick mark, it will automatically flip it if too narrow.
