Try edit the family. In the family editor, add your image

Once you load the family into the project, the image will appear and it is grey out.

Are you using Revit 2024? I tried this with 2022 and it seems to work fine. Try saving and renaming the family. Load the new family in and see if the image still appear.

I don't know what the issue is on your end. Everything is peachy-keen on my end.  BTW: Images only show on Sheets.

Ah! I see why. The one you posted is not the one you showed in your Screenshot.

When you load in the family, did you choose the second option? If you choose the first, it will not override the image parameter.

Well, it works on my end just fine.  No issues. Screenshot above proves it.  Are you sure you checked the box next to "Shared" in the Family?

I'm not following. The Image did not load into Project until I set the Family to Shared.  That "problem" Family you posted DID NOT have Shared checked and so the Type Image was "None" in the Project -just like your Screenshot shows. The solution is to make the Family SHARED.
