Form preview not given

I want to create a seat & would like to start with a flattened box form.  Unfortunately when I add the box I get no opportunity to flatten the box or change the number of faces.  Not sure what I'm doing wrong.  Check out the screencast.

my guess is that you are zoomed in too close to see the on-screen UI.  Can you try zooming out?  As for the dialog, have you ever used Fusion with a second monitor?  If so, try Reset To Default Layout:

Well I'm back...

Thought this was a zoom issue but it returned.  Tried resetting my view but that didn't work..
