Block icons (still) broken + viewport scaling is resized when attempting to insert block

Problem description:

1. When attempting to insert a block, block icons are (more often than not) missing -- most (if not all) icons are missing.  Running 'blockicon' does not always resolve issue.  Sometimes it does, more often than not, it doesn't  Is this a feature?
2. When fiddling with Scale / Rotation etc in the Block Palette, when I return to the main drawing window, a Paperspace tab, I find the viewport I am working in has been resized.  I had set it to 1"=30' AND LOCKED the VIEWPORT... yet the viewport scale is reset to a nonsensical scale.  Just now the scale has been set to 0.014634.  Phutt the whuck?  Is this a feature?  How to prevent this viewport re-scaling.  It's a PIMA and costs me time.

Regarding #1 above... block icons missing from the Block Tool Palette is still a regular occurrence and a PITA.  "blockicon" does not appear to fix the issue.  HOWEVER, running 'audit' and accepting changes does seem to "unblock" whatever is stopping up the piping under the hood.  After running 'audit', I have observed the Block Tool Palette re-populating.  If one is still having issues, running 'blockicon' is probably not a bad idea either.  Offered in the (vain?) hope of helping another chronic hair puller.

Still do not know why Viewport scaling is affected when inserting blocks whilst working inside of an active viewport.  I am trying to avoid that and only perform block insertion in Modelspace.  FWIW.

Are you referring to the Blocks Palette, or a custom Tool Palette.  Do you have any screenshots that you can share that demonstrate these issues you are seeing?  With it being 2021, which is an unsupported version, there will not be any fixes for it, do you know if this is an issue in the newer versions?    I've been using the Blocks Palette quite regularly for the last 3 years and I cant say that I've seen the issues you are describing.
