Ladder tool result in incorrect ladder diagrame

Hi 
When attempting to insert a ladder I get the following 

the template used is "ACAD_ELECTRICAL" 
Units are mm 

the project properties for drawing 

 
I tried the following widths and spacing with no success 
No                           Width                         Spacing 
  1                              20                                   2
  2                              10                                   1
  3                              40                                  4

the problem is I dont understand it why is it failing, some answers were about the graphics driver but how can autocad develop their tools to be this sensitive to the driver, I am also using tools like Inventor and never had any issues with graphics driver affecting functions. that said my grpahics card is Nvidia A2000 driver 528.05 

this issue has been going on for a while I tried to uninstall and reinstall no luck, I really need HELP!!!

Use a different template. ACAD_ELECTRICAL is drawing with imperial units. Try ACAD_ELECTRICAL_IEC instead.
