not associated elements

I am currently working in an engineer's model to try and fix all their orphaned and not associated devices.  For example, when i try to rehost a card reader it appears to host properly and i get no errors.  However if i look in the properties browser it says it is not associated.  It has done this with receptacles as well.  They are both face based items so I'm not sure why it keeps doing this.

In family template, model is not picked to the phase...ex: ref plane, ref line.

Hmm... post a small sample of your project so we can see this behavior and help you solve the issue.
