How to draw a slop reference plane

Hi, example i have two circle center point in different level, i want create a slop reference plane from two circle point and align to the center point and create the sweep .  The two circle distance would be change by different type.how to do it? Thanks.

do you need a "Reference Plane"  i would try a Reference Line for that.
