How can I specify/show the opening direction (innwards/outwards/left/right) of windows and doors in Revit 18?

Draw Symbolic Lines in the family editor.

You can draw them in the top or elevation views in the family editor using solid or hidden lines per your office standard. Don't forget to constrain them so that they don't go wandering all over the place when you adjust parameters. Remember - Symbolic Lines are only visible when they are parallel to the view.
