Some of the out of the box templates have a monolithic stair set up in them already....I started with the Construction (imperial) template and then duplicated it to create your stair....I had to convert the lowest landing to change the shape.  I attached the file if you want to do some additional testing.

You will need to edit the stairs and modify the landing height.

For fixing the landing, select the stair, edit the stair, select the landing and adjust it's relative height as needed.

If you select the stairs in either a plan or section (elevation would probably also work) you will have the option to add Rebar.

If you google how to add rebar to a stair you will find some youtube videos...I found the one below =)...hopefully that helps

Is anyone able to advise me why this monolithic stair is missing steps?

You've hopefully found the answer already by now! anyways it seems the splayed shape at the bottom of the stairs throws an error.

Deleting the shape and trimming to a normal yields a complete run without gaps.
