recessed-in-concrete lighting fixture opening

I have made a downlight lighting fixture which is recessed in a concrete slab, using the lighting fixture family template file, and checked on "Work Plane- Based" and "Cut With Voids When Loaded" in Family Parameters, but it can't cut the concrete slab in Project when I place it. The Opening option in 
Create tab is also grayed out in this template file.

I also tried to add a void object in the family with same options turned on, but it didn't work. It can cut the solid object in the family when I add the light to the Project, but not the concrete slab in the Project. It looks like it can only work at the 1st level (family) and not at the second level (slab in Project), (class inheritance design and definition in Revit software)

Is there any solution to make it into the family, or the only choice is to have a void object in the Project directly???

Oh, unfortunately I don't have access to Revit and that Family now, I'll upload it ASAP

Hi,

 

LightingFixtureOpt1, has a void object directly from the same family.

Lighting FixtureOpt2, has a nested void object.

In all families, "Cut With Voids When Loaded" option is turned on. 

it's Revit 2016
