Modify a extrude family

I would ask you kindly which command have to use to create the extrusion in the scrrenshot below.

Generally speaking, all family models contains mostly Extrusions and Sweeps either Solid or Void.

This model in particular contains a lot of Solid and Void Extrusions.

Capitel column model

Not pretty sure what are you triying to achieve on this, but the tools to build such as models are there int the familiy editor . The difference is the workplane wich is modified on the surfaces and in the case of this family they have nested another family for the top and the body of it.
