Label Offsets and Leaders not centered

Can anyone tell me what's going on here?  I've got a boxed annotation family, nothing in it but the box and the label, but the leaders won't snap to the center or the extents.  These labels/tags are from the default family templates.

Where is the Label and Box in relationship to the x,y Origin Ref. Planes?  

...just post the family here.  

...maybe it's referencing a Masking Region that you can't see.

Everything is centered.  Same thing happens without any box at all.

Yes, that is the problem.  But why is it there?  There's nothing in the family.

Add the label directly in the family instead of nesting it.

Aha, that got me on the right track.  The leaders got wonky without any lines in the family so I added a tiny invisible box in the center.  That got them in line and I could keep my nested family.
