You may want to take a look at this series of video tutorials on how to create a title block in Revit.

You can also import a DWG into a titleblock family template and then uset he "pick lines" tool to trace the element of your existing DWG title block. Then you can add labels etc... to get the elemtns whioch need user input.
There is no simple conversion too to switch a DWG title block inot a Revit title block family. The time you spend making a Revit version of the title block will be worth it over time.
