Not sure what I am missing from the csv file but every time I import the file revit the program crashes.

Check the regional settings of your system. Example see which decimal separator it uses: a comma (,) or a point (.) and what is used for digit grouping
You need to either match the formatting of the CSV file to your system settings, or change the system (Windows) settings so it interprets the CSV file correctly...Also, make sure to use the correct delimiter (comma, tab, or semicolon) depending on your regional settings and file format.
