15: Revision tracking/schedule

I've read a few posts over the past couple of days and nothing that I can find that would work.

I have a revision (tracking) schedule created.  Got it working until I add a second revision to a sheet and its listed under 'current revision'.  is there a way to show a sheet under each revision?

i.e. sheet A1 was revised 6/1/18 under Rev1 with a few other sheets.  Then when Rev2 is created, the A1 sheet moves from Rev1 list to Rev2 list.  

There a way to have A1 show up under both/all revisions?

feels like its a parameter or something that I am missing.  

ALSO - is there a way to add 'comments' to the revision could and have it added to the revision schedule?

Rev2015:

if I add a Rev2 to A503 (below), A503 will move from Rev1 to Rev2.  want to show it in both Revisions.
