The only time I see that is when there are overlapping polygons, like when you do an extrude and don't move anything. Try grabbing two vertices and moving them around, it might uncover the polys to delete. And then you can just snap the virtices back into place.
An alternative would be to make a cube, place the edge loops, then open up a side.
