iLogic Form Parameter Invalid

Hi, I have illogic form (3 rules attached to it) which popup when user create new IDW file.

Sometime everything works fine but randomly sometime when user create new IDW drawing, it gives the following error.

Any help would be appreciated as I need to fix this to remove this error from template.

Can you please provide IDW template along with iLogic rule and form to test and steps to reproduce exception? Please make sure that files are non confidential.
