Trouble applying sketch to cylinder

I have a cylinder with a hole in it.  I need to place a "foot" like object from a sketch and then join it.  Sorry as I am new to fusion and perhaps am butchering these terms.  Best to see.

 You can see how that sketch i would like to have perfectly extruded and rounded on the bottom cylinder?  I am at a loss on this one.  Been watching videos for a few hours and thought I would ask for help.

Like this one, Step by step up the timeline, read the settings by using Edit Feature on the ones you are not sure of,

 

My workflow was an extruded pipe, then a sketch of the Lug, on the origin Plane, easy to do there,

 

then extrude the lug out over the pipe, with two sided Extrude with New Body, make sure the whole lug goes over the pipe,

 

Then use Modify > Split Body, use the outside and inside faces of the pipe to trim the lug to size, and hide with the light bulbs the offcuts.

 

I filleted the top of the Lug, then mirrored it, cause you need 2,

 

Then Modify > Combine both Lugs to the pipe, Join,

then Fillet the bottom edges of the "Lug" all four with one selection,

 

There are a few ways to do it, but the two sided extrude did not work on the curved faces for me.

A variation on Günther's method would be to extrude the cylinder downwards, leaving the "feet" protruding from the cylinder.
