I am trying to add extrusion to an existing part.
It need to be placed a set distace from the top.
How do I measure down for placement?

Personally, I would create an Offset Plane from the Top of the Model (You can define the distance), then create a sketch of the feature that needs to be extruded. Extrude down toward the Top of the Model as Join.

 

Otherwise, can you export and share your model here so we can take a look at what you're trying?

Can you File>Export your *.f3d file to your local drive and then Attach it here to a Reply?

I doubt there is any need to create any offset work planes.

Your Sketch5 size and position is not fully defined.  You should fully define all geometry.

Sketch14 and 15 do not make logical sense.

Sketch16 is not fully defined.

Sketch24 is not fully defined.

 

Would you like to learn how to do all of this correctly with step-by-step instructions?

OK, let's get started.

Start a new file.

Start a new sketch on the YZ plane and orient the view cube as shown.

Sketch the two rectangles as show below...  (where did you get that strange 39.69624671 dimension?)

Are you sure that you didn't mean 17.7+22=39.2 because that isn't how your model measures.

You didn't follow my instructions about the placement of the rectangles.

Nevertheless we will continue...

 

Extrude the smaller rectangle symmetrically 88.7 as shown below...
