How to taper/chamfer part of circular disk edge?

There is probably a simple answer to this one but I haven't located it.  I have a circular disk of X thickness.  I want to taper part of the edge down to a thinner Y thickness.  Neither the chamfer tool or lofting seems to work.   I included a picture with the two thickness shown as a step; I want to get rid of that step and replace it with a sloped edge. 

 

Any suggestions would be appreciated.  thanks!taper.JPG

Remove the fillets on the ends, then use a two direction chamfer.  Add fillets at the ends.
