Block Properties Table help

Hi,

How do I create this test block in a manner in block properties table.

And How does the highlighted portion values work like. Anyone have example blocks?

Your initial values for the user parameters must match a row of the table in order for the check box to work.  See screencast.

Thank you.Can I control the output name here without changing visibility name? And What's the difference if I tick (block properties must match a row in the table) or not? And what does that Expression value do?
