Annotative Block within Dynamic Block

I'm working on creating dynamic blocks for my company, and I can't figure out how to make one work. I'm using a lookup table to change the width of the block. But I have an annotative block attached to the part that stretches that only moves at one scale and not the rest. How can I get the whole block to move with the rest? 

 

I've attached the block for reference.

You need to move rather than stretch the block.

 

You might want to reconsider the insertion point of the dynamic block.  It seems oddly placed off to the left.  

 

Also, because annotative blocks scale to/from their base point, you might reconsider it being in the center of the annotative block.  I moved it so that the block would stay 'connected' to the non-annotative bits.  See attached.
