Copy of an annotative scale appears?!

Often, I run into a problem where my viewports create a copy of an annotative scale. For example, I will copy and paste a viewport and change the annotative scale and standard scale to 1-1/2" = 1'-0" (added scale for our company), through the properties menu. As soon as I deselected the window, it creates a scale "1-1/2" = 1'-0"_1" (see attached picture). It happens on other scales as well, but mostly this one. Any idea where AutoCAD is getting this scale?

It actually had to do with the definition of our template scale list. The 1-1/2" = 1'-0", in our template, was set for 1 paper unit equals 8 units. However, AutoCAD sets the scales, with the 1'-0" designation, as X number equals 12 units. So the above scale should be 1.5 paper units equals 12 units. These separate definitions caused a duplicate X" = 1'-0"_1 annotative scale to be created. Once I fixed the incorrect scale, the issue did not reappear.
