Been using Autocad LT 2016 for years and just starting having issues with PDF underlay. When I insert the underlay in model space, sometimes I get the "image" and sometimes just the file pathway. If I save and close with "the image" and re-open - it is always the pathway. HELP, please.

Could be your graphics card driver
If you are getting the file pathway it usually means that the file doesn't exist at that location.
What I would do (I do do this actually) save the pdf in the same directory as the drawing. This is where it first looks for a file

@gesfire1948 - this post is being edited to remove PII.
Is it possible to post your drawing and the underlays so we can see if we have the same problem?

You may have to go to the graphics cards manufacturers website to directly check there though windows drivers may say it’s current. Also did you install 2016LTs service pack 1 update when it came out a long time ago? Do a screen capture of your AutoCAD about window and share that here
Does the same problem occur with image files or dwg files?
That is strange that attaching dwg & image files work but attaching pdf files just recently became a problem. I assume you’ve tested with all kinds of PDFs both vector and raster based?
@gesfire1948 click on the word ringed in the image below and then choose the files

@gesfire1948 is this sudden problem limited to this one PDF or inside this one DWG file?
Or is it now present if you attached other PDFs into other DWG files right now as a test?
