I have the MD layers created and linetypes set in my drawing template, but all of a sudden AutoCAD started making additional MD layers with _O at the end of them. I can merge them into the original layers but have no idea why AutoCAD is creating them.

honestly , i have NO idea how it's appeared . But you can by PURGE command get rid of them because it's still unused layers .

can you please upload the dwg-file where these layers are created, let us know the exact commands you are using + send us a screenshot of command/dialog _ABOUT so we know which product/version/update you are using.

I did that and i didn't get the _O layers, but now I have all my template layers (including the pre-defined MD_ layers) gone. That worked, but I lose my template layers and layer properties.
Maybe I'll try to purge them out of my template and recreate them.

Did your template already contain the "MD_" layers? If so this could be the source of the issue.
Try to purge all "MD_" layers from your template.
Then create one 3D-Solid, run one time command _VIEWBASE so you get the "MD_" layers created. Change the layer properties for these new layers to your needs. After that delete the viewbase-object from layout as well as the 3D-Solid object from modelspace and make this your new template.
Does that work for you?
