Snap Point Entity Issues…

I have recently run into issues with “Snap Point Entities” not working properly…
For whatever reason, the items I am creating, or have created, seem not to show up properly, or in most instances, are offset, thus resulting in a rendering error when importing into Navisworks or some other type of BIM software.
You really do not see the error until the rendering goes missing or shows the item missing. Upon a “recheck” into the original AutoCAD file you will find the entity is in the wrong spot however, upon clicking that entity, the “Snap Points” will appear in the right spot. (refer to attachments, or photos of “screen-views”.
As usual I just wanted to find out if there is any valid reason for this to happen or perhaps, if we may have missed an
update / hotfix through Autodesk?

Offset Location is shown correct but the "snap-points" is wrong
In this case , the "snap-point" is correct, but the wire frame is wrong
As you can see (the rendering) is correct however, since the "snap-points" were wrong the pipe is now in the wrong location
