When you File/open and browse to the .dwg files you are working on, about half of the icons have no color nor do they have the autocad icon. If you pick the box to the left of them sometimes they get their color but not always.

My first guess would be that those files are either an older version format or were created/edited with a non-Autodesk product, and that once you open and re-save them the icons will "update".
Once you open and save the files they look correct. One of my associates my be using a newer version of autocad. We currently save our drawings back to Autocad 2013. On Autocad 2018 you cannot do this.

If you are saving your dwgs back to 2013 version, then I assume this means you are using 2018 (otherwise there would be no reason to do that, since 2013-17 save to 2013 by default).
