Put the text on the line then use TEXTMASK, Wipeout; then Group them.

... or get a custom font with those shapes (arrow and heavy line) and drop them before/after your words.

To automate the process, you may be able to use my LabelTextTrim or LabelTextMask routines [the first BREAKs the object the Text is planted on, the second MASKS it behind the Text].  They include aligning the Text with what you put it on automatically.

MText justified middle center with a Background Mask set to 'Use drawing background color'  Set the MText at the mid-point of the line.

MTEXT has built-in masking.
WIPEOUT command is a core feature across all AUtoCAD variants.

If you are afraid to try the MTEXT built-in Symbols feature I pointed to above 🙂

If the Line and arrow are always the same length, you could create a block with an ATTDEF for the text.

Use the Multiline attribute feature and you are then able to have a background mask which automatically sizes to fit the text.

Try this for example.

Works for me in AutoCAD 2017.
