Doors and Windows are Massive

I am drawing with the annotation plot size at 1, which I assume means the scale is 1:1, but doors and windows are about 100 times larger.

Stairs are normal, and so are wall openings, but the doors are being strange.

Any idea how to fix this?

100x? As in centimeters to meters variation 100times? Either your blocks or your DWG file have mismatches "units". If you want to share your DWG file here, aka show your work, someone can confirm it for you if you cannot.
