CROSS REFERENCE TABLE

I'm trying to add a Graphic Font Format table to a drawing. I've done the setup to get the image on the right, but the version on the left is what I end up with. Anyone know why?

The two tables certainly don't look the same.  Can you attach both files so that I can better understand what you are trying to accomplish?

I want the one on the top, I keep getting the table on the bottom. the setup dialog doesn't work. I've done all the setups. they don't work. Solution?
