AutoCAD 2016: Insert Dialog Missing

I have an issue where the INSERT dialog is not appearing in AutoCAD 2016.

 

FILEDIA = 1

CMDDIA = 1

 

Any ideas?

I usually use the WINDOWS + LEFT ARROW option to move dialogs that are off screen, its not that.

 

The options appear in the graphics area as shown below, and the command line:

Don't laugh, I lost mine

I try .insert and it's back

 

DOT in front of insert.

Nope not that.  This works on one of our test computers via the deployment image, but doesn't on computers where our SMS package is installed. 

This may be an issue with the SMS package, and registry settings that we are pushing.

We updated our SMS package to have our customization loaded when double clicking a DWG file after installation, without the need to start first with a desktop icon.  We had to involve active setup, and were pushing all registry settings needed.  We missed a few registry settings, but have resolved the issue internally.
