why does vault /inventor create item with the same Position Number ?

we something have this problem 

in item workflow 2 files/item have the same Position number .. This cause an error  when we Export it to our EPR later .. why does this happen an how beste to fix it ?

Did you open the assemblies structured BOM in Inventor?

well I did that later... renumbering the position number.. but I want to understand why it happen in the first place and if possible make sure it never happen again 

it is still rare but it seens to  seens to happen more often now.  I am not sure if we even have the problem 3 months ago.

Unfortunately, I don't have an answer to why it is happening. Vault gets the position number from the Inventor Structured BOM information. So it starts in Inventor.

Is there an illogic code for renumber it ?...then it might be fix by simple force renumber on save
