Really you would need Autodesk Construction Cloud (ACC) as it is made for this type of application but unfortunately it does come at a cost!!!
This would allow you to upload your models to the cloud, open the model and sheets on a tablet for instance and then you could assign Issues and Markups onto the plans and models in the views where possible anomalies arise. You can add text and photos to the issue and it even noted the location/coordinates and these can are directly linked to the Revit Issues Addins to be addressed later. You can even create reports and copy other member in the Project in so they are aware and receive email confirmation.
I have seen where a PDF was used on a tablet and they just put highlights of Red and Green and circled number to note information and related notes but this is a little basic and additional notes are still required to log differences.
if you wanted to use Revit you could add a Parameter to the elements for checking with a Yes/No function and set up a View with a Filter for Yes = Green, No = Red and select the element and tick/untick the box but obviously this would be quite long winded and still an amount of user input is required.
In an ideal world it would be great to walk around the structure where each element has a barcode/QR scan and this gives the information of the member and if the coordinates are correct it correlates with the Model on the tablet and marks as Green!
