Face-based Family Annotation Not Visible on Horizontal Face

I have a face-based floor box family which has an annotative symbol linked in. When I place the family on the floor, the annotation is not visible. I have identified that this is solved by extending the extrusion up above the 0" plane so that it enters the view range. My question is whether this is behaving as it should and I just need to keep a piece of it above the floor or if there is something weird going on with the family itself. I have noticed that the host face within the family is dashed and my extrusion (which is fully beneath the floor) is dashed as if they're both "beyond" my view range.

The second issue I'm having is with the visibility parameters of the annotation itself. The DUPLEX floor box annotation will not turn off. All of the others operate as expected, but for some reason this one won't turn off when the parameter is turned off. It appears to work fine within the annotation family, but gives issues in the floor box family and, in turn, in the my project's model.

In Revit 2022 you can set the family to Maintain Annotation Orientation.

Am I missing something? How would that help my issue? I have it inserted into a 2022 model and that is selected, but I am having the same problems. I don't have an issue with annotation orientation, just that the duplex annotation will not hide. It's an odd occurrence to me because (if you open the file you'll see this) the annotation family is pretty crude with all of the symbols being overlapping lines in the same family (it takes a minute to separate them and discern between them).
Maybe the answer is to separate my symbols into individual families and use the "visible" parameter for each family instead of self-created parameters. Although it shouldn't matter, should it?

It looks like there are lines within the annotation family that I cannot locate. When I tried copying that family to create individual annotation symbols, the phantom duplex floor box was still visible. If anybody wants to dig in and figure out where it's coming from, you're more than welcome but I just recreated the annotations from a raw family template and it's working as expected now.
As for the visibility in my floor plan views, I just extended the extrusion to +1/8" to imitate a trim/flange so that it penetrates the view range. I wish view range was inclusive of the limits, but I'll have to keep this in mind for future families.
