Window tag center of window

I am tagging windows in elevation and some of the tags work great and point directly to the center of the window while some of the others don't even point at the window at all. All of the tags that don't point at the center are the same window family so I'm assuming this must be a problem within the window family? I can't figure out what the relationship is between the window family and the tag family.

Are the tag leader endpoints Attached or Free?

they are attached. I just included those to illustate where the tag is pointing to. I would like to have no leader and for the tags to be placed at the center of the window automatically. This is working for the other window families but not this particular one for some reason

Where's the "Origin" Ref. Planes located at in the Family? Always on Center of the Window?

There are two muntin generic model families nested in your window family and it appears only one is being used.  If it is the case then delete the other one and the tag should center. 

After deleting the rogue muntin.
