Unable to create a form error

Hi

Greetings of the day.

I am new to Revit and it's my 1st post in Auto desk forum. I am trying to create a massing form for a facade look. The steps i followed are 

1. in- place mass created 5 reference plane.

2. By using spline drew the model as in attached screenshot (south view & 3D view ) 

3. By  selecting all clicked on create form, its showing an error of unable to create form.

i am not able to resolve the issue and please let me know what mistakes i have done.

Waiting for a quick resolution.

shown in my revit attached image before.
