Remove gaps in staircase railing

Any idea how I can fix the gap I have in this stair railing? And also why these corners are not joining properly??

One thing to try. Get rid of the Railing Definitions and use Top Rail instead.  I think you may find the transitions better that way. I'll return some other suggestions tomorrow. It's late here.
