Leader with Annotation Head

I need to create a leader in Revit but rather than having text, I need to have a circular head. I have included a picture of what the symbol looks like. It is an annotation symbol that has already been created in AutoCAD but I need to make a new variant in Revit. Thought it would be possible if I could possibly create a custom leader and load in the head, which I have already made. A picture has been included. Any help will be much appreciated.

If you make the head a generic annotation family with labels then you can add leader to it in project.

....also, when you insert the leader, select it, and go into its properties, you can control the kind of arrowhead you need per office conventions.
