Filled Regions/Hatch Issues

I'm having issues with filled regions and cut hatches showing incorrectly. When I cut a section through our foundation system which includes a structural slab on grade and grade beam, the concrete cut hatches are showing incorrectly. Please see snip below:

The same goes for the Granular fill filled region I added below the grade beam. There seems to be some kind of scaling error?

I've tried cutting new sections and redrawing the filled regions and adjusting the scaling of the cut fills and filled regions.

Has anybody experienced this before? Interestingly, the drafting views that I have included as a part of our typical details are not seeming to have the same issue. It is only happening with the details that I have created specific to this job.

Revit has a  maximum distance limit from its Internal Origin of 10 miles. Beyond this limit, the graphical representation of elements becomes less reliable and less accurate.  Could this be your case?
