Hi how to go about it ,
1-Make a copy of the section view toy are working on.
2- In the copied instance, put the display, on hidden lines ( adjust to the same scale as the copied from )
3- draw the filled region on the copied version
4- group select all by filter, then exclude the filled region you want to show. Now you should see only the filled region
5- place the hidden line view you've just created over the realistic view on the sheet.
Now you have a realistic view showing the filled region detail.

Your drawing in Realistic mode is actually a render of the 3D model only showing the material finishes., whereas a filled region is a 2D tool that will not show up in realistic mode.
We have few users reports on similar issues in views whose visual style is Realistic, filled regions are displayed as solid black or solid white instead of the specified color and fill pattern. This has been resolved with the latest updates.
Please verify version of Revit are you using?
