Family visibility and view orientation

I am having a problem I cannot figure out. I have a face based electrical fixture family(attached) that contains a nested, labeled family to control which cover plate family is used. I also have a clearance box object that is just modeled in the top level family. Both the nested family and the host have their visibility settings adjusted to show only in elevation views(images below).
The problem is, when placed, the family will only show up on surface that are oriented north/south (assuming north is 'up' in plan). The box object modeled directly does show up in all views, all the time.I know the nested family is set to be invisible in left/right views and that's what I want. I've literally made a new project, created four walls in a square, placed this family and I only see the nested cover plate in the two parallel north/south elevation views with all overrides turned off. It's like the visibility option is hiding the family in views that are oriented to some kind of global east/west left/right. I've tried messing around with true/project north and it doesn't seem to matter.
Have I just mucked up the visibility settings in some way between the two families?
