Address Parameter

I am using Revit 2013 and I am working on creating an address stone and Would like to know how to create formulas to  get these model text numbers to change to the address that I type in. I know there is a way to do it because I have done it before, but I recently switched jobs and now have to recreate this and cant remember how I did the formulas! I have included a picture of what I am trying to do...
