Extrude face and edge only has 1 arrow controller

What's up with this wack extrude face arrow?  I already reset the tool setting this has been going on for a while and I've been shift moving instead to extrude but I need the good ol extrude back.

Do you have a live surface enabled? (Screen Cap included of little magnet to enable/disable)

@cavescholar while make live has an effect on the extrude manipulator it doesn’t change it’s appearance.
