Rig Controller moved off mesh

I've been working on an animation assignment and recently found that the right arm controller for my rig moved off the mesh and is floating out of position. I'm not sure what caused it, but all of my incremental saves now have the same issue.
 
I've tried resetting my preferences, deleting the controller's history, and restarting my computer with no luck. I included a screen shot so you can see what I'm talking about. The blue arm controller is supposed to wrap around the top of the arm. 
 
Is there anything I can do to fix this?

I have the same problem and was wondering how it was fixed?
I have an assignment due on soon and have no idea why my controller shifted or how to fix it so an explanation would be reallllly appreciated thanks!

I attached a screen below. The eye controller on the right is supposed to be hovering over the face but it's wayy up above my character
