Need Help with my arms

Building a modified humanoid rig, and just started on my arms. I'm at the stage where I move the orientation of the IK handles from the Twist Joints to the Wrist joints. This is supposed to be incredibly easy: select the IK handle, press "insert", and v-snap to the joint. I've done it dozens of times while rigging, and way more than that with other objects. Should take about 5 seconds, tops. Sadly, every once in a while, while rigging, it goes squirrely. Instead of must moving the orientation point like it's supposed to, it starts messing up the skeleton. The only way I've ever been able to get around this is to either start over from scratch (groan) or get someone else to fix it and send it back to me.

Can anyone volunteer to sort out this mess?

Oh, I'm still using 2016, because I could buy it, and hate Subscription Only Software (SOS), so some form of down-save may be needed.

Did the file not get attached?

I got weird error messages about application octet stream, which makes no sense. Maybe this site no longer recognizes older Maya file formats. Let's try again.
