How can I create a Prompt Entry Window like this when I start a new drawing?

No really need for code if your just using drawing iproperties.

If your drawing template has fields that are defined as 'Prompted Entries', a pop-up box will appear when you start a new drawing.

Prompted Entry is the easiest way, but you wont get collapsible sections. 

Place a text box in your titleblock and choose prompted entry:
