When i add icons to the quick access toolbar, sometimes they stay, sometimes they do not. I like having the Vault check in icon on the QA, so i add it while in an assembly and most times it is not there when i open the next assembly and i cannot re-add it.

Do you have the Factory Design Utilities installed and loaded?
If so try switching off the "Load Automatically" and restart Inventor 2023 and then see if it remembers your icons

I did have a situation where the reason my edits would not stay was that the Factory add-in was loaded, I can give you a long list of steps you can follow to get them to stay in your Quick Access Toolbar. The strange part is some tools would stay and some wouldn't, so it is selective of the ones that get reset. I have logged this issue with Autodesk and they are hoping to have a patch out to fix it soon.
Right-click on the ribbon and select “Customize User Commands…” from the Part environment.
Add any commands to the right-hand side that you want to be in the QAT (eg check-in, check-out and iTrigger)
This adds a new “User Commands” Panel to whichever TAB you were on when you clicked on “Customize User Commands…”
Now Right-Click on these one at a time and add select “Add to Quick Access Toolbar” and they will all be added to the QAT
Now you can hide your “User Commands” Panel by Right-Clicking and selecting “Show Panels” and then selecting “User Commands”
You will need to do this for all environments ie: Drawing and Assemblies

You could try resetting your ribbon and then see if the issue is solved as it could be an issue with settings that have been migrated from a previous version. Note this will reset any customizations you have done previously. My workaround for Factory may also work for you though so if the "Ribbon Reset" doesn't work maybe try those steps?

We just rolled out 2023.2 to our department and have a couple of users that are experiencing this as well. We don't have any of the "Factory" stuff installed so I don't have the option to unload. We did not try doing the ribbon reset as my user's didn't want to loose anything. They did not do any migration of customizations from 2022 to 2023. Just posting to see and get notified of any updates that may be needed.

It brought to me attention that some of the mysterious framework-level issues could be related to the way the dlls are loaded. By default, Inventor loads dlls on demand to boost startup performance. However, in some cases or some machines, this performant loading process can lead to inconsistent command behaviors. Here is an environment variable to add to the machines. It only alter how the dlls are loaded. Essentially, by disabling the dynamic loading, all dlls are loaded on startup. It may cause minor slowdown on startup. But, I don't think the difference is that much in modern PC, particularly with SSD drives.
1) Close Inventor.
2) Go to Control Panel -> System -> Advanced System Settings -> Environment Variables -> add "DYNAMIC_LOAD_APPLETS" and set it to 0.
3) Restart Inventor and perform the problematic workflow.
It may or may not help. But, this is something to try.
